Outreach Support Worker

At Headway Worcestershire, we support people living with acquired brain injuries (ABI) to lead fulfilling, independent lives. As an Outreach Support Worker, you’ll be at the heart of that mission — providing personalised, compassionate support in people’s homes and local communities.

What You’ll Do

• Support adults with brain injuries to live independently and confidently.
• Help with everyday tasks such as managing appointments, daily routines, and community activities.
• Provide emotional support and companionship.
• Encourage social inclusion and personal development.
• Monitor wellbeing and update care plans and reports.
• Work closely with families, professionals, and other support services.

What We’re Looking For

You are:
• Empathetic, respectful, and genuinely care about making a difference.
• Able to work independently and manage your own schedule.
• A great communicator with strong listening skills.

You have:
• Experience supporting vulnerable adults or people with additional needs.
• A full UK driving licence and access to a car.
• Basic IT skills for writing reports and accessing support tools.

Even better if you have:
• Knowledge of brain injuries and rehabilitation.
• A Health and Social Care qualification (Level 2 or above).
• Previous outreach or community support experience.
